Patient Surveys

Patients are contacted via a random selection process every three months to participate in a Patient Care Survey for patients over the age of 15 years, conducted by the
Health Quality & Safety Commission New Zealand.

Please let us know if your email address has changed or needs to be updated.

It would be great if asked, you are able to make time to complete this upcoming survey.

The information gathered is used to monitor and improve health services nationwide for the benefit of patients, so feedback is very important.

Before you visit:


Telephone consultations and face to face consultations are still available
Please phone 03 315 6328 for an appointment.
When phoning for an appointment, please tell the receptionist if you have any respiratory symptoms:
new or worsening cough
sore throat
shortness of breath sneezing and runny nose
loss of sense of smell or altered sense of taste, or a fever
headache
contact with person who may have been exposed to COVID

When you visit:

We politely request you still wear a mask if you display any resipratoy symptoms, this helps to keep the wider community safe.
You may still be asked to be seen via th Ambulance bay if there is any doubt on the type of infection you may have.

Continue to practice social distancing

Covid vaccinations are still available.
Please call the Health centre to arrange a day and time for your vaccine.
