Patients will have an opportunity to complete a short satisfaction survey here at the health centre, running for a month until December 20th  2023. 
If you have an appointment during this period, you will be offered the chance to complete the survey at the time of your visit.  If you are not attending the clinic during this time but would like to take part in the survey, copies will be available at reception.
The results and feedback for the team will help us to measure ‘what we are doing well and what we could do better’.
All results will be anonymous.
If you would like to be entered into the draw to win one of two prizes, either a free skin check appointment or a Hanmer Springs Thermal Pools & Spa voucher, complete the separate prize draw slip with contact details and place in the box provided.

Before you visit:


Telephone consultations and face to face consultations are still available
Please phone 03 315 6328 for an appointment.
When phoning for an appointment, please tell the receptionist if you have any respiratory symptoms:
new or worsening cough
sore throat
shortness of breath sneezing and runny nose
loss of sense of smell or altered sense of taste, or a fever
headache
contact with person who may have been exposed to COVID

When you visit:

We politely request you still wear a mask if you display any resipratoy symptoms, this helps to keep the wider community safe.
You may still be asked to be seen via th Ambulance bay if there is any doubt on the type of infection you may have.

Continue to practice social distancing

Covid vaccinations are still available.
Please call the Health centre to arrange a day and time for your vaccine.
