COVID-19 Patient Experience Survey

You may be randomly selected to take part in this important survey about your health experiences over the last few months. This is a national incentive post the COVID-19. The pandemic has changed the way health care has been delivered and your feedback will help us understand and improve patients’ experiences.
This voluntary survey should take around 10 minutes to complete, depending on your answers.
Your responses are anonymous and will not be connected to you in anyway. Please be open and honest in your answers.
This survey will look at health care since the COVID-19 lockdown began in New Zealand.
It does not matter whether you accessed any health care during this time, all feedback is still relevant.

Before you visit:


Telephone consultations and face to face consultations are still available
Please phone 03 315 6328 for an appointment.
When phoning for an appointment, please tell the receptionist if you have any respiratory symptoms:
new or worsening cough
sore throat
shortness of breath sneezing and runny nose
loss of sense of smell or altered sense of taste, or a fever
headache
contact with person who may have been exposed to COVID

When you visit:

We politely request you still wear a mask if you display any resipratoy symptoms, this helps to keep the wider community safe.
You may still be asked to be seen via th Ambulance bay if there is any doubt on the type of infection you may have.

Continue to practice social distancing

Covid vaccinations are still available.
Please call the Health centre to arrange a day and time for your vaccine.
