Community Services Card (CSC)

If you hold a current Community Service Card, please let reception know, so that the details can be updated.
If you think you may be eligible for a card, you can find more information and forms at www.workandincome.govt.nz
Patients with a current CSC will be eligible for reduced consultation fees. Procedures continue to be charged at the relevant rate and are not subsidised through the CSC.

Before you visit:


Telephone consultations and face to face consultations are still available
Please phone 03 315 6328 for an appointment.
When phoning for an appointment, please tell the receptionist if you have any respiratory symptoms:
new or worsening cough
sore throat
shortness of breath sneezing and runny nose
loss of sense of smell or altered sense of taste, or a fever
headache
contact with person who may have been exposed to COVID

When you visit:

We politely request you still wear a mask if you display any resipratoy symptoms, this helps to keep the wider community safe.
You may still be asked to be seen via th Ambulance bay if there is any doubt on the type of infection you may have.

Continue to practice social distancing

Covid vaccinations are still available.
Please call the Health centre to arrange a day and time for your vaccine.
